Please complete and submit this entire form for your new hire.
If we use Ease for your enrollment, we will reach out three times to enroll them. We will notify you when we do so. After three times and no response from the employee, we will assume the employee does not want to be enrolled. We will enroll employees in the employer 100% paid benefits you may offer. When the employee is enrolled we will provide you with an enrollment summary for the payroll deductions.

- Benefits will begin according to your waiting period and will generally begin the first of the month following the hire date and waiting period.
- Example: First of the month following 60 days would look like this:
- Hire date: Feb 10th + 60 days = April 10th
- Benefits would start May 1st
- Example: First of the month following 60 days would look like this:
- The hire date you provide is what we use in the enrollment.
- We cannot enroll outside of the waiting period, these are set by your carrier.
